9/14/2023 EnchantaVaughn Approved Behavior: Client was present in the home when her counselor pulled up. Client communicated she was in a great mood. Client had on a nice outfit with a new set of braids. Client normally keeps herself in good order. Client was witnessed adhering to the rules and regulations in the home and giving mom assistance with anything she needed done around the house. Client was applauded for her teamwork skills and presenting excellent behaviors. Behavior: During the second part of the session, client needed minimum redirection and instruction and was able to maintain appropriate conduct and express positive thoughts about the session. Client remained in a happy mood for the remainder of the session. Client did not need any further support with managing her symptoms, due to displaying excellent manners and high self-esteem throughout the day. Client was suggested to take a small break before going back to the home setting in order to cool off and present relaxation and calming skills. Client presents pleasant behaviors while transitioning back to her normal routine. Intervention: QMHP went to the home to meet with client individually. QMHP engaged in conversation with client about her day. QMHP actively listened to client verbalize that she was agitated today and the reasons to her agitation. Instruction, modeling and role-playing techniques were used to help client’s general communication skills. QMHP taught client effective problem-solving skills (i.e. identifying the problem, brainstorm alternate solutions, select an option, implement a course of action, and evaluate). QMHP informed client that when she is agitated to refrain from taking it out on people and communicate to others, so they know exactly how she is feeling. Intervention: QMHP engaged the client in a role reversal exercise and required the client to think about her goals for the future. QMHP modeled proper positive behavioral expectations for the client to encourage additional layers of support and understanding of rules and expectations. QMHP provided mom with at home strategies to practice with client in case the behaviors that persists become uncontrollable or overwhelming and she’s not able to handle them any longer. QMHP encouraged mother to allow client to express herself in an age appropriate manner and to redirect when needed. QMHP applauded client for her improvements. Response: Client discussed mental illness and its causes as a family. Client displayed interest in learning about ADHD. Client blames her mental illness for her attitude, mood, and behaviors. Client stated that she doesn’t intend on being negative and agrees she need to work on controlling and channeling her anger, as well as, listening and communicating her feelings better. Client displayed management of individualized behaviors. The client has not overreacted with anger to minor frustrations or irritants. Client responded in a pleasant manner acknowledging the positive reinforcement that the QMHP provided. Client was able to reevaluate herself and display a more positive attitude toward the authority. Response: Client have adapted and is observed modeling appropriate behavior. Client wished she had more friends at her new home and plans to start building more relationships with her peers once things go back to normal. Client was happy and in a good spirit throughout the entire session. Client displayed positive expressions and maintained good tone when communicating with QMHP. Client was respectful and polite, utilizing acquired skills provided by QMHP to share her ideas when answering questions during one on one. Client was seen following directions. Client and her mother reported that they understood the importance of working towards her goals and objectives daily.
